Vegetarian diets and children's nutrition: doctor guide

The short answer: A well-planned vegetarian diet can completely support normal growth and cognitive development in children. However, because plant-based foods have lower bioavailability for certain key minerals and lack natural vitamin B12, vegetarian parents must deliberately plan for four critical nutrients: vitamin B12, iron, vitamin D, and zinc.

1. Vitamin B12: The non-negotiable nutrient

Vitamin B12 is synthesized exclusively by bacteria and is naturally absent in unfortified plant foods. In lacto-vegetarian diets, dairy products (milk, yogurt, paneer) are the primary natural source. However, research shows that boiling milk repeatedly reduces B12 content by 30% to 50%. A vegetarian child who drinks minimal milk or eats little curd is at significant risk of B12 deficiency, which can cause anaemia, lethargy, and neurological symptoms. Regular supplementation with a multivitamin containing bioactive B12 is strongly advised for vegetarian children.

2. Iron: Bioavailability vs Total Intake

Vegetarian meals in India (dals, chana, rajma, palak, methi) often contain generous amounts of total iron. However, this is non-heme iron, which is bound by dietary phytates and oxalates, yielding an absorption rate of only 5% to 8%. To boost plant iron absorption:

  • Add vitamin C (lemon juice, tomatoes, oranges) to pulse dishes.
  • Soak and sprout whole grams (moong, chana) before cooking.
  • Keep dairy milk and chai separated from main meals by at least 2 hours.

3. Vitamin D & Calcium

Calcium is usually well-supplied if a child eats curd and paneer. However, calcium cannot be absorbed without vitamin D. Because neither vegetarian nor non-vegetarian foods contain substantial natural vitamin D, regular sunlight exposure or supplementation is essential for bone mineralization.

4. Zinc & Protein

Plant-based protein combinations such as dal-chawal (pulses + rice) or roti-dal (cereals + pulses) provide complete complementary amino acid profiles. Adding roasted chana, nuts (almonds, walnuts), seeds (pumpkin, sesame), and paneer ensures adequate zinc and protein intake.
